Did you know that Community Supports often are available through your managed care plan if you are enrolled in Medi-Cal? Community Supports are services designed to address health-related social needs, help people live healthier lives, and avoid higher, costlier levels of care. Some examples are:
Air filters and ventilation,
Home modifications,
Transition assistance from nursing facilities or homelessness,
Accessibility improvements,
Establishment of utilities
People enrolled in Medi-Cal may receive a Community Supports service from their managed care health plan if they meet the eligibility criteria. Their managed care plan also must determine that the service is a medically appropriate and cost-effective alternative to services covered under the base Medi-Cal program.
